I am delighted to acknowledge the support and encouragement that I received from a considerable number of colleagues over the course of this study. Over the years my wife and family have become accustomed to my academic preoccupations, yet they have constantly made allowances beyond what I should have expected. I thank them again. This book is dedicated to my grandchildren. They bring so much joy.
